Make a Plan Setting a good goal is an important step toward successfully making changes in your life. Good goals are small and specific. Small steps with steady progress over time lead to big results. Make your goal specific and measurable so you can easily see if you are truly meeting your goal. Be aware of potential barriers that could prevent you from making the change. Be flexible. If your plan isn't going as planned, make an adjustment and move forward. Try not to dwell in the past. Make sure your goal is realistic and achievable and then write it down. Tell Someone Support and encouragement play a very important role in behavior change. Let a friend or family member know you have set a goal to improve your health. They might even join in your effort and you will be able to motivate each other. Tell your health care provider. He/She may have helpful advice and also be a voice of encouragement. Reward Your Success Plan for success and don't forget to reward yourself before moving on to the next goal. The reward can be small, it doesn't have to be extravagant. Get a massage, plan a special game night with friends or family, buy that thing you wouldn't normally get for yourself. Take time out or do something for yourself in honor of your accomplishment. Ask For Help If You Need It You don't have to go it alone. Self-Management Tools The following tools can help you to see proof of progress toward your goal or show you when or where adjustments need made. This information is also very useful to your provider and may help him/her to make a decision about your treatment plan. Bring your logs into your appointments to review with your provider.
