Openings Patient Facilitator Care is provided to patients of High Plains Community Health Center on a patient centered basis characterized by excellent customer service, timely appointment scheduling and rapid visit cycle times. Performance of all duties with patient except what the provider will do clinically. If an open position is not currently available, we will maintain your application for six months and consider it for future openings. Primary job duties are to work closely with medical and dental teams to provide integrated behavioral health care to patients.
