CICP High Plains accepts and registers patients for the Colorado Indigent Care Program. This program provides discounted health care services to low income individuals at participating providers. CICP is not a health insurance program. The program is funded with federal and state dollars to partially compensate participating providers who provide health care to the uninsured and underinsured. The Colorado Department of Health Care Policy and Financing administers the CICP. Health care services are provided throughout the state by hospitals and clinics that participate in the CICP. Not all hospitals and clinics participate in the program. HPC Sliding Fee Scale High Plains Community Health Center also offers an in-house sliding fee scale to patients who may not meet all of the criteria to qualify for the CICP program. While CICP is accpeted state wide, the HPC Discount program is only available to patients who access the care provided at any High Plains location. Covered services include medical, dental, and dispensary. Women's Wellness Connection Free cancer screenings for uninsured women: Age 40-49: Free pap, pelvic exam & breast exam Age 50-64: Free mammogram, pap, pelvic & breast exam Eligibility: Income & age eligible, uninsured or underinsured, one year since last pap or mammogram.
Health Coverage Guides High Plains Community Health Center has Health Coverage Guides on staff to help patients navigate the changes to insurance in 2014. Health Coverage Guides can assist patients in applying for Medicaid, as well as enrolling in commercial insurance. Chronic Care and Health Disparities Collaboratives To eliminate health disparities and improve functional and clinical outcomes, High Plains has changed the way we deliver care. High Plains has special programs for patients who have diabetes, cardiovascular disease, asthma, depression or oral health needs. We follow specific clinical guidelines for treating these illnesses and contact our patients for necessary care and testing throughout the year. Same Day Appointments High Plains offers same day appointments at all sites. Phones turn on at 7:45 a.m. so call to be seen today. Sick Call for All Ages Been up all night with a sick child or feeling ill yourself? Now you can know first thing in the morning if you can send your child to school or you can go to work. Walk-in sick care Monday-Saturday, 8-9 and Monday-Thursday 5-7:30 pm. Saturday Hours High Plains offers Saturday hours 8 am - 12:30 pm, 1:30 pm - 5 pm. Phones turn on at 7:45 am. Free Pregnancy Testing Walk-in to any medical site during normal business hours for free testing. Registered Dietician Having a healthy diet is one of the most important things you can do to help your overall health. Food choices play a role in energy level, bone health, weight management and risk for heart disease, diabetes and some cancers. Meet one on one with our Registered Dietician or attend one of our FREE evening classes. Health Coaching High Plains is the only local provider of FREE Health Coaching services. Your health coach will assist you in personalized goal setting, offer helpful tips and connect you to local resources setting you up for successfully achieving your goal. Each site and provider team has a health coach available for your convenience. Drug, Alcohol, and Tobacco Abuse Counseling We provide screening, counseling, and referrals for substance abuse. For more information, contact Lisa at 719-336-0261. Free Colon Cancer Screening We offer free colon cancer screening to all patients age 50 and older. Come in for your flu shot to receive your packet. For more information, call Shae at 719-336-6976. Integrated Community Health Partners Also known as ICHP, this program has been established by the State of Colorado to aid Medicaid members and their families in accessing the right care at the right time. Contact Crystal at 719-336-0261. Reach Out and Read This program promotes early literacy and school readiness to young children and their families. A free book is given to children through the age of five when they come in for well child visits. Well child exams should be done at 1-2 weeks, 2 months, 4 months, 6 months, 1 year, 15-18 months, 2 years and then once per year every year there after. |
