All year High Plains Community Health Center offers $5 Sports and Camp Physicals, as a service to the community. Use the check list below to get organized and ready for your child's appointment. Ready? Ask about a FREE exam. Insurances and Medicaid now pay 100% for annual well child checks/physical exams so schedule your free exam today. If you are uninsured, we can do your child's physical for $5. Fill out your forms. Please have your form signed and filled out completely before your appointment. If you do not have a form get one from your school or camp, pick one up from any High Plains CHC location or download now. Schedule your appointment. In an effort to provide the best care possible please request an appointment with your primary care provider. Each provider has allotted times for $5 appointments. To schedule, call 719-336-0261 or request an appointment online through the patient portal. SET, Before you leave your house for your appointment... Collect your forms and money. Make sure you wear or bring your glasses/contact lenses for the eye exam. DON'T go to the bathroom, we will collect a urine specimen in the office. Arrive on time for your appointment... Let the greeter know what your appointment time is and which provider you are going to see. Give your forms and $5 in cash, check or credit/debit card to the medical assistant that takes you from the lobby to the room. Take your paperwork with you.... After your exam is complete make sure to take your signed physical forms with you. PLAY!!!! Stay active and best of luck! |
